Cypress Healthcare and Rehabilitation is hiring a Assessment/MDS Nurse.   The  MDS Nurse plans, coordinates, and helps monitor the resident assessment and care-planning processes in the facility.  MDS Nurse works with the Director of Nursing to ensure the highest quality of care is provided in a cost effective manner to the residents.  Must hold valid nursing license.  Our full time employees are eligible for the following benefits:
